Can someone tell me why I might be getting this error?

  I seem to have fixed that one... and caused a new thing...
  
  My beautiful new form, that is SOOOO easy to complete is located at http://elevatingyourbusiness.com/session/use/ElevatingYourBusinessSessionForm/form1.html
  
  Now I get this one (and yes, I did answer every question, including the ones that I didn't have to. )

  Well, I still can't figure out which of the 3 or 4 optional items wants an answer ;)
  
  Possibly somoene can help? I am still getting an error message that says

    Yes. I ended up a bit frustrated and re-did it. I have people testing it now.
