Fields are missing from new form

    Hi, I have been struggling with this for days and can't seem to get past it.  I have created several sample/test forms successfully using all the features I need in my site forms (text box, drop down list, text area, email fields) in all of the templates to see what they looked like to see what I wanted to use, as well as make sure I could achieve the forms creation.  All had 5 or 7 fields and worked perfectly, including the email field.

    When I go to create my real formagain , which contains over 40 fields but nothing unusual, the fields disappear - the rest of the form is fine but no fields. The form, table, etc. was there and even the email worked fine, there were just no fields either in the final form or successfully sent email.

    Upon trying the form from scratch again, I noticed that when going fron the first to the second page after filling in the field information I got a 404 and a406 error (browser does not recognize it as a web page)  - I'm using IE7, and I think this may have happened with the previous form and I just refreshed the page and the next pages came up and I continued on.  I did not allow this the third time, I did get the first page to FINALLY accept when I clicked the "next" button but the same error happened with to the next page, and gave up trying to get to the next page and just refreshed, and there I noticed the fields were not listed at the bottom of the page (I specify to have the script create the table and when it works the suggested column titles for the fields show on that page.)  Same result, no fields but the empty form worked fine.

    Does anyone out there have a solution for this? Does the form creation have a timeout feature?  My browser is working perfectly otherwise and I'm having no issues with my web host provider either.  I'd appreciate any suggestions, thanks.

      There is no timeout feature.  I'm not sure exactly what is happening but I am sure I can get you going. 
      Do you want to send me a link to your form and an email of the process.php file or do you want to just try and work through the forum?

      I was ASTOUNDED by your instantaneous response - I was actually thinking this might not be the way for me to do forms (this is my first) but you have definitely restored my faith!!  Thank you so much!

      I've been up all night but when I'm not asleep I should be attached to my computer all weekend, FYI   :-(

        Don't have the email yet but it does not mean I won't get it eventually.  I have some pretty unusual email delivery rules when I'm not at home so some things get unusual delays occasionally. 
        All I need to get started is a link (URL) to your form.  If I need more detail I will need you to send me the process.php file inside an email (this file can not be viewed on your web site).
