Ok, I'm not very familiar with any of this (being I'm a windoze person)
I installed this thru fantastico.. get it installed, configure my first form, etc, etc,

Finish and give form a try and I get the following:

Try to (Change Permissions to) chmod 755 all files and folders inside the folder you choosed to install.

      Example:
      phpform/*.* including phpform

the fast way is trought the ftp but u can do inside your cpanel your cpanel ->file manager-> click in the folder and than click -> Change Permissions

      Nelson:

Thanks for the input, I checked and it shows all folders for the forms set at 755, some of the files are set to 0777

      any ideas?

I also put in a ticket at my host, see if they can come up with anything...

Hello, I have the experienced the same error on my installation. My host was unable to give me anything helpful.  Were you able to resolve it with your host?  Any information that anyone can share would be very much appreciated.

I solved this problem using Nelson's advice, it seems that my hosting does not like folders set to 777, (i experienced similar problems with a shoutbox installation). so basically, se all the folders and files you can find to 755 and you should be alright. even just the one folder set to 777 appears to mess the whole thing up.

      Hope this helps!

I configured the forms folder to 777 and also it appeared that error message. I followed the Nelson's tip and configured for 755 and nothing changed. What's wrong? Please, help me!

      Sorry for my bad english.

Msg to Nelson: Are you brazilian?

You will have to provide a link to your form and send me your process.php file (email) before I can help.
