I just installed this program. Created my form. Renamed it and copied to my top level directory on my homepage. When I launch the page it comes up, but prompts me for the administrator password. What did I do wrong?

I figured it out myself. I just copied the "folder" directory to my root directory of my web site and it work. THe key here is to make sure you copy all the files under the "folder" directory.

Once everything was installed and I created my first form (form1.html). From MS frontpage, in the Remote Web Site window, I drilled down until I could see the "forms" directory. I then dragged the directory to the "C:\documents and Setting\... (my top level directory where "index.html" resides.
To test that it worked, I just created a hyperlink on index.html to the form1.html within the forms directory.
