1- I'm not getting to generate the form. In the last page, when I need to insert some information like hostname, username, password and database for mysql I don't know what to write in hostname and password.
I would like to know where i find these informations.
2- I did another way (don't check the generate database option) and i don't need to insert those informations. However when I visualize the form, insert the data and send it, appears this error message:
"Internal Server Error
The server encountered an internal error or misconfiguration and was unable to complete your request.
Please contact the server administrator, webmaster@deliverybr.com.br and inform them of the time the error occurred, and anything you might have done that may have caused the error.
More information about this error may be available in the server error log.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request".

Answer the question of whether or not you need MySQL first before you worry about filling in these options. If you chose to use the mySQL option for your form the generator will setup the mySQL db for you. The only option you may not know how to answer is the hostname. In most all cases this is "localhost".
The other information you chose is arbitrary names and passwords that identify the db that the generator will create.

I think I need a phpformgenerator tutorial :( I don't know absolutely nothing about phpformgenerator, mysql and php :/
