Is it any preparation for database needed before i run install.php like table creation or any other such things. Thank you.

Sometimes when you copy your script to your server, scripts run as different users.  Sometimes this username and password are not sufficient for access to your SQL db in which case you may need to at least create the table name before the install script will finish without error.
I am just guessing based on past experience with version 2.09 where I have had this issue.  I am not sure if this issue has been completely fixed in version 3.0. Are you experiencing a specific problem?

Hello, It is appears working GOOD now! I just try it with both sending to E-mail and to Database and it working very well.
with sending to Database option it generates"config.inc.php" automatically when i run "install.php." and report this after running.
After this, when i run "form.html" it works great and added inputs to Database and also send me an email !

                                          IT IS GREAT,
                                           IS IT? ! ! !
Also, when i chose send data to email, it works great and send me an email.

Best of All, I did'n do any thing else upload form files to my server and run the "install.php" !

YOU HAVE TO TRY IT AGAIN FROM SCRATCH, JUST LIKE I DID.

                                                 Thank You

Important Note related to files permission !
After Uploading your files to the server you MUST change your main form folder and also the folder named "files" inside it, you MUST change there permissions to "777" , this is before you start to run "install.php" file, other wise i don't think that you will get good news! :)
