I've recently moved all of my client websites to Lunarpages because our previous host had too many outages. However Lunarpages has blocked the use of any mail programs such as formmail, which is why I tried your phpFormGen today.
First I tried Fantastico but found there were too many problems. There's no links to the pre-made forms even to check them out, and no other documentation, and so I came here. Nothing I was doing was working, not even the advice through the forums here (deleted .htaccess, CHMOD, etc.).
I deleted the Fantastico version, and uploaded all of the files from the one offered here. I CHMOD'ed and set up the files, going through the README file as well as the setup instructions provided on the webpage (by Mark).
Now with this new version, nothing is working. I set up a new form with 4 fields. Not even the test scripts included will work. CPanel says "premature end of script headers" and the page I get when trying to use the form is a 500 Server Error.
Lunarpages is wonderful, but is this a problem with their server? It seems unlikely since they encourage using PHP and not formmail. Is there any further documentation? Any files in particular that I should be going through to check?
I'm at a loss here, I've installed hundreds of scripts and never have I had this sort of trouble in getting one to work. Usually I can find at least three different things to try in the support forums, but here it seems everyone is having the same problems without results.

Thanks anyway, but I decided to go with something else that I knew would work for sure. I'm sure you've written a great script that is divine once it is configured and working, and maybe I'll give it a shot another time, but right now this one isn't right for my needs.

The claim that everyone is having the same problems without results is not true. There are over 20,000 users who are enjoying the program without any issues. Ofcourse, you get to see problems stated in this help forum, because why would people ask for help if they get the program working perfectly?
The problem is definately with your hosting company. Infact, that is the problem with many large hosting companies ... they limit their users to such an extent where they can't do much of anything at all. You can try out Microterra Solutions, which caters to client needs on a personal level... something no big hosting company will ever indulge in.
Finally, I am not sure how the program doesn't meet your needs. Assuming you were looking for a php form generator, it should pretty much be able to do everything. This program has been reviewed and featuered in a number of computer magazines, bundled on CD-ROMS, and recommended in many web master tool suites. The popularity of the program is due to the fact that it is extremely easy to use, requires very little computer knowledge, and yet creates powerful and highly flexible forms which have capabilities such as database connectivity and file upload. Furthermore, even the installation of the program is now quite simple. If the program is not configured right, the program actually prompts you with error messages to go fix what is missing. Of course, there's nothing much that can be done if your webhosting company explicitly puts limits on things ... in that case you can either live without using popular tools, or you can switch to a different hosting company which actually caters to your needs.

After putting the program into my server outside of the public_html folder (cpanel), the docs indicated that it should be in the public_html folder. So I moved the program into public_html from where it was. Now going to the directory tree given in the "Installation Docs" I get the following message:
