I have created a form in formgenerator and have customized the html so it has the same feel as my site. Now everytime I try to go to the form it asks for phpgenerators username and password. I want anyone to be able to go to this form. Am I suppose to move the form to an unprotected directory? Also I wish to change the password of phpgenerator. Is this possible? Any help would be apperciated.

phpFormGenerator does not come with any sort of password protection. Try downloading a fresh copy and installing it on your webserver. Everything else should work fine. I am guessing that some of the webhosting service providers are adding a .htaccess file for security which is ending up causing problems.
