Last night I accidentally installed the form onto my main domain site www.fortunate-girl.com, and now when I try to access anything it always asks for the username and password, even though I uninstalled it. Is there anything I can do to make it stop?

Did you figure out how to fix this problem? I am currently having the same problem. I installed the formgenerator and now I cannot view any of my site's pages without the username and password for the form generator. I tried uninstalling the script, but it still does the same thing.

to remove the admin password thing,when u enter the website, what u have to do is uninstall the php form generator
then log into your filemanager, and your index page file name has changed, change it back to index.htm
and should be fine, but yeah i have the problem when u click submit on the form it asks for admin and username password, any help would be tops, thanking you

Same problem in a different way.
I installed the software, built a form, placed links to it, etc. When I try to access the form from:
[url]http://www.floridaarsonseminar.com/forms/forms/form1.html[/url]
I get the admin screen first, then log in, THEN get the form. I should not get the admin box at all.
Any Ideas?

You might make sure the form directory and all subdirectories are set for read+write access. If you use Linux/Unix set the permissions to 777. Good Luck!

I have a similar problem over at www.eze-cash.com
I have some lovely forms set up but whenever anyone clicks on the apply now button on the page they are prompted for the username and password before they get to the form that is to be filled out?
Could someone have a look at the site and they will see what my problem is and please provide me with detailed instructions as to how I might fix it?
I am completely new to all of this and this is the first site I have built on my own. (used dreamweaver MX for most of it)

sweet i fixed it thanks skipwathen, in cpanel for my hosting i got a password protect thing in the directory i had a look, and my folder with the script was protected, i unprotected now its fine
