I have installed phpformgenerator 2.9c with Fantastico and spent three days getting it to work correctly so here is my advice to anyone else. I use cpanel(9) and don't know if other cpanels will be the same.
First time installation should work fine using this method. If you have previously tried to install the program you should be sure all traces are removed, databases and all files.
1)Install with Fantastico create directory and complete installation.
2)chmod forms directory and all files in the directory to 777
3)access cpanel and click on Password Protect Directories (PPD)
4) the directory for phpFormGen (for this tutorial I will refer to the directory as phpFG)or whatever you named it should be locked. click on the name of the directory, check the box and save to unlock.
5)return to PPD and access again. click on the phpFG file icon to open files.click the directory  forms (name), check the box to lock. If no active user shows below create one giving a name and password then, click add modify/authorized user. go back and save to lock forms directory.
6)after you create a form ,return to PPD and access, click on the phpFG file icon to open files, click on use file icon, click on the file icon of the form you created, click on the admin (name), check the box, create user if none exists (as in step 5). This must be done to keep admin protected from others via the net. You must lock the admin for each form you create.
7. There is one more file that should be changed.
Access the phpFG folder via file manager or FTP, open the folder and see index.php, the name of this file should be changed to (whatever you want.php) for added safety. Just remember what you named it so you can access the index to create more forms.

One day the makers of this program will put the index.php file along with the other exposed files in an Admin folder then they can be password protected as well. I tried putting them in a folder and PPD but, something needs to be changed in the script.
