the form folder Contact does not exist. Unless you moved it or changed permissions, the folder appears to have been deleted.
You should also consider some minimal level of security for your forms and your form generator. For example; if I follow this link http://www.srgproperty.com/forms/
I can ctrate forms on your host. I can also delete forms on your host.
Use the tools available from your host to password protect these folders.

I just checked my website and clicked on one of my forms and got a 404 error. After checking the rest of the forms I have the same problem.
The only change I made recently was to MySQL data base.

Thanks for that tip. I'm discovering each day how much i forget.
Have been wrestling with why several of my forms keep disappearing at a couple of installation/sites i've developed.
i knew it had to be the work of a troll.
know i know why, i gave them the key! ;)
