If you point your browser to the URL below you will see that this file can not be found. This is the file that creates the Captcha images. Until this works you will never get Captcha working. Either you did not copy this file to your site or it is not working for some other reason.
If you point your browser to this file and it does not produce an image then this is the source of your problem. Below are the requirements for this file to work. Run these requirements by your Web Host tech support to see what they say. Give them the URL to the file.

If the requirements were all met then you would not have a problem. I am 100% confident that this page produces a captcha image all by itself. Every time you refresh you get another image. This is the best test for whether or not captcha images are successfully working. Your form just makes a call to this file to insert an image into your form.
Maybe its a file or folder permission error. Without access to your site I can not do much more for you. All I can say is that "I know this works" when all the requirements are met.
