I have generated a form and uploaded it to my site but the form does not send the results to the e-mail I specified.
please advise!
The form is here: http://www.fhci1969.com/form119827/form.html

If it is a problem, the evidence will be in the process.php file not the form file so there is no way to evaluate this issue by looking at the form.
If this form is being used as is (as it was generated) it is likely not an issue with the process.php file but more likely an issue with the web host PHP installation/configuration.
1.) Contact your host to discuss the issue.
2.) If that does not work then come back and be prepared to share your process.php file so I can see what it looks like.

I have looked in the processor.php and the mail part looks fine.
I don't know much at all about php, but, do I have to do something in the "where_form-is at" line? should it point to a specific server?
I do not mind sharing the file - there is nothing confidential - it's an RSVP form.
How do I share it with you?
I will speak with my host and see if they need me to do something ... and let you know...
thanks!
