Installed phpFormGenerator via Fantastico in my new hosts cpanel.
When I installed it is told me an extra step was required to protect the admin area and I had to supply a username/password.
I installed in a directory like so www.host.co.uk/forms.
I could not get it to work, what should I put for hostname when im setting up a form, also at the last step even though I told it to create a new table it still asked for the fields to be filled in on the last page.
The worst problem is that when I tried to access my website it asks for the username/password for the admin area even though I am trying to access the index page.
Even worse I unistalled the script via Fantastico but it still asks for the username/password when I go to my home page even thoughI have uninstalled the script.
Any help would be greatly appreciated

Fantastico's security step is problematic. It puts your entire site in a deadlock. Try to find a .htaccess file in your webserver directories and remove it. That should get rid of your username/password issue. Do not install phpFormGenerator through Fantastico. Check the news section of this project, I have already announced this. You should download a fresh copy of the program from this site and install it yourself.

Also, if you cannot find the .htaccess file you can restore a backup of your site from before you installed the script. It should fix the problem, it worked for my site.
The second time I installed the script to my site I used Fantastico and skipped the final step and everything seems to be working okay so far.

I have the same problem. I did as suggested in the other post, but still can't access my site. I run a webzine and it's very important that I get this problem sloved. So far, nothing's worked. Please, please help if you can.
