The authorization issue is because you installed phpFormGenerator through Fantastico. Don't do that. Uninstall it from Fantastico, and then download the program from here and install it on your own. That will fix your authorization problem.
For #2 and #3, it is wierd that its doing what you are describing. I'm guessing its due to something that your webhosting provider doesn't allow, and thats why the program starts acting funky. You can consider Microterra Solutions: http://www.microterra.net , webhosting with 1GB of disk space is provided for only $5/mo. The servers are reliable and cost effective, and they come with free official phpFormGenerator support. Which means, the staff there will install a fully functional copy of the program for you, so you don't have to do it yourself.

Im asked for authorization.
Well, I want this to be accessed by my visitors.
I tried to change permissions. By the instructions, all boxes should be checked except fpr group and world for write permission.
So, that changes to 755.
I have tried to solve this and sometimes the authorization required windows pops-up others wont.

The second problem Im having is when I test my http://www.pro-ecovers.com/ofor/use/order/form1.html page after filling in ALL the fields, I get this error message:
The following errors occured while processing your form input.
You did not enter one or more of the required fields. Please go back and try again.
Again, all fields were filled not only the required.
3)
When I change some parameter on any of the drop down menus I go to another (nonexistent) page.
Please advise on how to solve this. Please understand that English is not my main language besides te difficulty on this software language.
