I have never been able to use this thing

I have never been able to use this then whenever I have tried it.

I always get a blank screen after I submit the form.   The email never gets to me.

OR

I create a database, a username and a password in MySQL account maintenance, in CPanel.   Then I use this information to create the form, and it stops me here.

Now creating table 'formz' ...

Could not access MySQL database with information provided.

So.....
