I'm a novice at best so treat me lightly.

Recently updated the company's website, and included a response form as provided by phpformgenerator 2.09c. Unfortunately the form works for about a week or so and then ceases to be found http://www.acleigh.co.uk/responseform/use/form2/form1.html. Its still there on the server, it's happened a couple of times. Where's it going, am I missing something?

Thanks in advance for any assistance.

Try removing the generator.  Try movoing the form to a new location.  Make sure the form has some basic security (read only html anf graphics, read only php if your host supports this).

I had a similar problem where my use folder kept getting deleted.  I deleted the del_form.php file and have not had a problem since.

Your suggestion is not very productive.  Deleting the file will prevent the tool from ever deleting any form created.  This breaks a basic function in the generator and I do not suggest this at all.  The del_form.php file still needs input from another php file.  Executing this file by itself does nothing.  The cause of the form disappearing is not related to this file unless someone runs the generator, selectes the previously created form, and clicks on "Delete".  This will never happen by chance.  Someone must be driving, hence the need for basic security.
