    NAPNES - 2007-02-18

Having troubles with my form not working.  Can be found by going to:

    http://www.cmastaffing.net/forms/use/provider_registration/form1.html

Getting an Internal Server Error message.

Installed using CPanel.  Any help is appreciated.

I know this borders on an insulting question, but did you reset the permission for the files - at least for form1.html and process.php?  I also loaded phpformgen via cPanel and I change the permission of the form folder to 755.  The form folder is the one that bears the name of the form you created. (This is an example of the root for a form I created for my company's website, as it would appear on smartFTP.  The form folder is Cutoff_RFI and the tree looks like this -> /public_html/thenameofmywebsite/requestinfo/use/Cutoff_RFI

I have found the easiest way of doing this is to download smartFTP (it is free) and then go to the website in smartFTP and navigate to my form folder.

I then right click on the form folder and type in 755 in permission window. If you follow this technique, be sure to click the checkbox that asks whether you want the PERMISSION to be applied to all of the files in the folder.

If you load smartFTP, you can create the form, then got to it on smartFTP, change the permission (if 755 does not work, try 744) and then fill out the form and submit it.

      Good luck.

Since it was installed with cPanel I assume it was made available from your host.  If this is true then the first step is to contact them and let them know it is not working.
It looks like the file can be found (process.php) because you don't get the standard "Page not found error".  It looks as if the host does not recognize this is a php file and therefore does not know how to treat the file.  PHP relies on a server side interpreter to identify and handle the php code before the result is displayed or used on a web site.
