here's the address to my site's contact form.

    http://hmvillage.com/site/index.phtml?action=contact

if you fill it out and click on the send button you'll see what i mean. It keeps on giving me error 401. And when ever i click on the send button i have to type in my pass and username. Why does it do that? (It only works when i type in my username and pass, and i don't want it to do that.)

Does anyone know why? I just want to use it contact form on my site, so that the viewers on my site can use the form to send comments to me.

BTW, i have tried everything. I tried 777 every folder and files in my forms directory.

It is because you have installed the program through Fantastico and you haven't searched the help forums completely! this has been discussed several times over and over again. Infact, it is also mentioned in the project news section.

oh i'm sorry. i didn't know that fantastico had anything to do with the error.

okay so now i download formgen and now do i just upload all the files into a dir. and then go to forms/index.php to install it?

I'm afraid you are not doing much research on your own before asking questions. Have you visited the project homepage? http://phpformgen.sourceforge.net
There is an install guide right there, it has everything you need to know. Please look around before asking your questions. It just clutters the forums when people ask the same question again and again.
