I did contact my host and found there is a tool in their admin panel, CPanel, for managing access to directories.
This access method uses .htaccess. I was close to having it working, but had the wrong directory for the password file.
Anyhow, I got it to work so that access is limited to the admin page for phpFormGenerator.
This is a great program.
By the way, my host is http://fluxservices.com
