When creating your form, make sure that you are not putting any special characters such as : , ; and so forth. Only alphanumeric characters (0 thru 9 and a thur z all cases) are allowed.

I have the same problem. I'm not putting any special characters.
On the step one screen I enter my number of fields. No matter what number I put in I get the following message on the step 2 screen:
Notice: Undefined variable: error in D:\Inetpub\wwwroot\dogrir.org\html\phpform\fields.php on line 174
I have yet to successfully generate a form.
