Need Help with Form Submission

    I set up this form a couple of days back (performance review) and uploaded to the server.  I just need the results sent to the email, and I have the confirm.html set (I see it in the php code).

    When I test the form and hit "submit" I get a blank page.  No confirmation, no email.

    All associated files and folders are in the original locations as when it created.

    Can someone help out with this?  I spent a lot of time setting up this detailed form the other day.

      Submition goes to the process.php file so the problem is with that file.  Unfortunately I can not see this file.

      FYI - I also looked in the code in my form.html page, and at the php file in the folder.  I did not change these. 

      I have no process.php file, only one called processor.php, as it appears in the html code:

      <BR/><!-- begin form -->
              <form method=post enctype=multipart/form-data action=processor.php onSubmit="return validatePage1();"><ul class=mainForm id="mainForm_1">

      All of the forms I have used for the past two years (that are currently working) all have processor.php (not process.php).

      I think that the problem is with installation process, so, see next post 1st and then reinstall your form again and i hope it will be fine

      Hi, Essam - thanks for the reply.  I've never had to change the permissions to anything when using these forms in the past. 

      Can you please be more detailed in where I do this?

      I'm not sure what I must change the main folder and "files" folder to?  Please explain.
