You are right, this is really messed up. I can help you fix it but it's going to take some effort. Let me know what you want to do.
If you choose to start over be sure to create the form step by step as close to the order you want the form to appear in. The less you move items, delete add, etc the better.
This may be a bug in that the generator somehow looses track of the field type and location mapping when generating the final form. I can not be 100% sure without going through the exact process you did when you created this form.
