I'm new to this gendra....I installed a form and it looks great. Installed a second form and went in to edit it and got a message that I needed to give "read+write permission to all files". I went back to the README files, but it is empty--just a blank page. Where do I go to assign permission to edit the forms? And where can I find the README contents?

Forms don't give messages like this. It must be some kind of system message from your web host. You'll have to provide more detail otherwise I am not sure what the real problem is.

On the phpFormGenerator page it shows a new installation option. Below that, it shows current installations and it lists the two forms I have created. Beside each form name, it gives two options: Visit Site and Remove. If I click on visit site, I get a page titled phpFormGenerator Installation verification. It says "you are seeing this page because you did not follow the steps mentioned in the README file that comes with this program. This page is for installtion verification and will ensure that you ahve performed all the steps reuired for this program to work properly. Please take a look at the following message and fix any errors.
File and Directory permissions

The forms/for1.html file is not writeable.
Please give read+write permissions to all the files and dictories mentioned above. Refresh this page after you have done so.
On the page that gives the instructions and parameters for the forms, it says to do this under the phpformgenerator directory. Where do I find this, since the README file is blank?

This file is in a folder that is used to temporarily hold the form you are creating. If this form is not writable by the generator you will never be able to save your newly created form. When you get to the last step you will only be able to copy the read only copy of this temporary form. You must correct this.
When you install the generator you get the following folder structure.
[install folder]
[forms]
[admin]
[files]
[templates]
[use]
[your forms by name, initially nothing]
The forms folder is where the file is from your message. Go to this folder and make all the files RW (664), and all the folders RWX (777).
When you create a new form the new form is stored in the [install]/[forms] folder. When you finish the stored form is copied into the [install]/[forms]/[use]/[form name] folder.
UPDATE: ok-- I erased everything I had done and started all over again. This time, when I finished and clicked on "view site" it took me back to the origination page to create another form. Am I correct, forms cannot be "edited" mut must be re-created if there are changes to be made?
Also, is there a way to insert a "security lock" since I am asking for credit cards? Thanks for your help.

You are correct, version 2.09 does not allow editing after a form is created. The idea was that the generator creates the basics and editing was left to the owner.
I created a version that stores data files such that a form can be edited after it is created. This version I called 2.10 but it is not supported by this forum due to the release of version 3.0. If you would like to try it send me an email with a request and an email I can send it to.
I am not sure what you mean by security locks. I assume you have a specific need for taking CC numbers. Usually the CC number is taken for purchases. The charge is made using the full number but the full number is not stored even in your db. You need to take security very seriously if you are asking for CC information. I suggest you review your requirements carefully and if you are not sure of something that you get some professional advice (talk to your bank or your merchant account representative).

I appreciate your help. I went to my website and to the files you indicated, but cannot find where or how to change them to RW ( 664 ). Is it a format or a formula, or is there someplace I should be looking for this option?

How do you access your site? Is it through a web host supplied control panel or through an ftp program? Either way you should have the tools available to change permissions. The method may also depend on if your host is Windows or Linux based.
There is no way for me to profide specific instructions without knowing the tools you use to access your site.
I use WSftp. There is a free version called WSftp-Lite. If you used a tool like this one then the procedure is just right click on th efile or folder and select properties. All the permission changes can be made from this properties dialog.
If you can not figure it out contact your host for assistance.

Forgive me for breaking into this message...I thought I may save time for both of us by doing so... I am having issues as described which I think might be similar:
"Warning: opendir(use/) [function.opendir]: failed to open dir: Permission denied in /home/EDITED_OUT/public_html/phpform/copyfunc.php on line 28"
I am wondering if I also have done something wrong with permissions. I have checked several times, and all files have been given 777 permissions. In the Forms Folder...all folders and files are 777. Is this correct?
New related topic...having all files as 777..what security issues are there, if any?
Thank you, nti
