I copied and pasted the form's source (from form1.html) into dreamweaver and made my desired changes, then opened the "process.php" file and added/subtracted whatever fields, etc. I had made, so it reflected the newly edited fields in the html file. You also need to edit your mySQL db table to add or subtract the fields that were changed.
