Problem with error message

Basically I reshuffled the code to include the form in my webpage and now am getting an error message that one of the fields is not complete.

Any suggestions I have moved the process php file, global include file, fields php to the correct directory to so there shouldnt be a problem.

without seeing the code it is hard to help, post your html page, global.ini.php and process.php
