You could specify a stylesheet and any layout surrounding
the survey in the template file (the file that you added
the <?php ... ?> tags to). A very small example (hope your
mail reader doesn't try to interpret the HTML below):
<html>
<head><title>custom title</title></head>
<body>
<center><p><img src="customheader.png"></p></center>
<table width="90%"><tr><td>
<?php $sid=1; include(...); ?>
</td></tr></table>
<hr>
Some Copyright (c) or Something.
</body></html>
Does that help? You are free to do anything you want around
the PHP tag, including specifying a stylesheet etc.
