[phpesp-general] some fixes and limit double postings

Hi all,

for all of you to enjoy: I've submitted some patches to phpESP 1.8.2, see:

And for those who would like it:
I've changed handler.php (the same code can be re-used in survey.php) to
limit double postings through the use of a cookie. It can be configured
in the phpESP.ini.php file, and is disabled by default (so double
postings allowed). If people are interested in this, I can post the
differences to handler.php.

Again for those interested: I changed some of the code: I added a file
public/phpESP.first.php that needs to be included in all php survey
pages you show on the web using handler.php.
The reason for this: phpESP now uses a cookie to prevent double postings
(if wanted/configured), and using handler.php within a page prevented
the cookie from being set correctly (because headers already sent to the
browser). I worked around this with ob_start() first, but that was an
ugly solution, so now I created phpESP.first.php, that does this, but it
needs to be included at the top of every php page you use to show surveys.

[phpesp-general] Cookies Patch (ver. 1.8.2d) & Public Results

I'm using Franky's patched version 1.8.2d and using the cookie feature 
to block repeat voting...

However, in virtually all of the polls I am using I like to have the 
results public, and right now I'm at a loss on doing that, as with this 
version in place, once one has voted you cannot view the results if the 
cookie is still valid - It indicates you've completed the survey already!

I tried working around this with a 2nd config. (ini file) and patched 
files that seemed to be affected, however that doesn't do any better.

The result is the same if I call with $results=1 in the PHP handler, or 
if I use the modified survey.php uncommenting the $results setting.

Suggestions?
