[phpesp-general] not able to submit survey.

Hi,

I have just installed phpESP and might have something
wrong which I am not able to figure out. I am not able
to submit any form/Survey that I have created.

Here is a test survey having that problem.

https://www.telcom.arizona.edu/wq/dev/phpESP/admin/manage.php?where=test&sid=5

Can somebosy tell me what I was missing.

I have installed phpESP-1.7.5 yesterday.

In the management interface, change to 'Activate' and then 
access the survey with this URL:

https://www.telcom.arizona.edu/wq/dev/phpESP/admin/manage.php?where=results&sid=5

select a couple of questions, and hit submit.
