Unless someone submits a patch, I wasn't planning on expanding question
types for a while. I would like to get the GUI and survey rendering up to
current standards and looking nice. Which leads to your second question. =

don't think it is possible to change the layout as you've requested without
a good bit of code changes. Once tables are removed from the survey layout
and we use pure CSS, you should be able to accomplish what you want just
with a CSS template.
