Summary: bad return link
Initial Comment:
the 'return ' link which appears after a survey is
complete works fine after user has completed one survey
but if the user has several surveys to complete,
subsequent displays of that link drop the name=xxxx
querystring parameter and thus lead to a 'survey not
specified' error page
I am using the survey.php?name=x method for displaying
surveys.

Use a page linking to the survey.php?name=x page, then the
redirection will be to there, and work as many times as
necessary.
