Summary: Return link on confirmation page error

The "Return" link on the confirmation page only works
the first time a survey is submitted. On the second
and subsequent
tries the survey name is left off the link causing an error

The intention of the "Return" link is to go back to the page
the user came from *before* the survey. (It takes the
referrer passed by the browser.) If you have a page on your
site that has a link to the survey, then when the user is
done they will be returned to *that* page, not the survey
itself. If you type in the url into the location bar, then
there is no referrer, which is probably the problem you were
having. Any further discussion on this point should be
carried out on one of the mailing lists.
