This is normal, due to some errors in the current version. Please try
the version on my page:

Make sure to read the changelog. In the main dir there are some
examples on how to include the survey in other pages (surveu.php,
survey_nl.php, survey_fr.php).

(I know, I know ... getting to the CVS this weekend I hope ;-)
