[phpesp-general] some fixes and limit double postings

Hi all,

for all of you to enjoy: I've submitted some patches to phpESP 1.8.2, see:

And for those who would like it:
I've changed handler.php (the same code can be re-used in survey.php) to
limit double postings through the use of a cookie. It can be configured
in the phpESP.ini.php file, and is disabled by default (so double
postings allowed). If people are interested in this, I can post the
differences to handler.php.
