I'm encountering several issues. I'm able to install the module on to my PHPNuke website and create a survey, but when I click on the phpESP link in the modules block I get the message "[ Error processing survey: Survey not specified. ]". If I use the link that was provided at the submission of the survey, I can access the survey and complete it. When I try to get the result of the survey via the admin area, I get "Error opening survey. No responses found. [ ID:3 ]" although I received the email with the results. It's even worse on my PC, where I have a test server. I'm able to install the mod, but when I click a link in the admin section to create a new survey (or anything else), it just refreshes the page to the phpESP main page.
