[phpesp-general] Survey not fully posting.

I'm having a problem with surveys fully posting for the end user. About
1-in-5 to 1-in-10 page loads fail to completely load. The first half to
three quarters of the page will load, but the rest of the questions as
well as the submit button are missing. Refreshing the page often but not
always fixes the problem. 
Any assistance would be appreciated.
