Are you using IIS?  Are you using PHP in CGI mode?  Either of those 
cause authorization to break, turn off 'auth_design' and 'auth_response' 
in the phpESP.ini.php file.

A good start at more details would be the test page output 
(http://www.example.com/phpESP/admin/test.php).
