[phpesp-general] Checkboxes putting the results in CSV file mult times

We are testing a survey, and one of the results had all check box 
results entered 4 times.  For example, if the check box had a value of 
"Credit Card"., the csv file contained "Credit Card, Credit Card, Credit 
Card, Credit Card"

I tried to reproduce the problem, but was out of luck.

This survey does contain multiple pages, and has some questions marked 
as required.

Does anyone have any ideas?  Thanks
