Justin,

You seem to be using the php-nuke version of phpESP.  The php-nuke 
version was made by Eric Esselink of nettrade.nl.  I am not involved 
with the php-nuke version.  However, it does seem that what you need is 
to specify a survey name as in:

.../modules.php?name=phpESP&surveyname=foo

Perhaps other people on the list may be better able to help you with the 
php-nuke version.
