[phpesp-general] phpESP help

I am having problems with phpESP here:

www.teenymcavote.org/yvotemembers/ under "TAKE A SURVEY" in the menu

but I don't know why or how to fix. It also looks like there may be a new 
version out too-- but I don't know how to install. I talked my web-host to 
install one, but only after much begging because they have told me many times they 
don't offer support to do that.

Also, it looks like you all are responsible for making this -- I have to say 
this is by far one of the coolest, and greatest contributions on the internet. 
Especially for us smaller nonprofits who really benefit from surveys and 
stuff -- and students.
