I think a request for a question type similar to what you've requested is already a RFE: http://sourceforge.net/tracker/index.php?func=3Ddetail&aid=3D934740&group_i= d=3D8956&atid=3D358956 Unless someone submits a patch, I wasn't planning on expanding question types for a while. I would like to get the GUI and survey rendering up to current standards and looking nice. Which leads to your second question. = I don't think it is possible to change the layout as you've requested without a good bit of code changes. Once tables are removed from the survey layout and we use pure CSS, you should be able to accomplish what you want just with a CSS template.
