Summary: mailed results off my one for rate1-5

Initial Comment:

For a question of type "Rate Scale (1-5)" the results
in the admin window are fine, but the email that is
sent has one added to each result.  I.e. 2-6 instead of
1-5.  

Unable to reproduce with demo site (no email sent) but
here is

Hrrrmm, funny.  I run a survey with ratings from 1 to 10 and it 
lists one less  (ie. 0-9)

I ran into this issue as well and filed a bug report a month or 
2 ago.  Basically I think that the app databases values from 0-
4 rather than 1-5.
