[phpesp-dev] Rate questions

Currently when CSV exporting rate questions, an answer from 1->x it will
be exported as 0->(x-1), with N/A's represented as -1.  Should this
remain or be changed?  I think it's more logical to export as 1->x with
N/A's as 0.
