Great Survey Tool

1) Is it possible to include the number of the required 
question missed?

2) Text/directions at the top of 1st page only.

3) Is there an ability to include images?

The original RFE was for an easy way to modify/specify the
redirect link already exists on the first survey creation page.

Answering the followup RFE:
1. not possible
2. sure just add a section text question to the top of the
1st page.
3. only what is possible using the available CSS themes.
