Summary: "other" option with text entry

Initial Comment:
Often "Other" is included as the last item in a list of 
options.  It would be nice to have space to type in a 
value when the user's choice is not available.

e.g.:
What is your favourite colour?
[ ] blue
[ ] green
[ ] red
[ ] other: _________________

This feature is already part of phpesp 1.6.1.  Kindly 
visit /admin/manage.php?where=new:

For check boxes and radio buttons, you may enter "!other" on 
a line to create a fill in the blank option. An "Other" box 
defaults to using the prompt Other: , but is configurable by 
using the format: 


!other=prompt text

phpESP can already do this. See !other in the docs. and on
the survey creation pages.
