Summary: shift values for rank question type

Initial Comment:
When viewing or downloading results for surveys, I see
admins getting confused by responses to 'rank'
questions. The confusion is that a suer gets to choose
between 1...x but the responses are stored as 0...x-1.
This mismatch coudl be easily avoided by storing the
responses starting at 1 instead of 0.
