Summary: Adds file-upload capability

Initial Comment:
Used the commented-out question-type number "7".

If that is a problem, can change to number 10.

Adds file-upload capability, and attaches the uploaded
file(s) via MIME attachments to the response email.

Haven't yet integrated this with the survey reports.

Possible enhancements would be to explicitly list (via
the question options) the acceptable MIME types for
uploading.
