I implemented cookies/cookie decoding by integrating phpESP into a portal (I 
use MyphpNuke). When someone is registered on the portal they can access and 
fill in the survey and once they complete it they can view the results. The php 
coding I used also recognises if they have already filled in the survey so 
they can only fill it in once. Using cookies is a good way of doing what you 
want If you have any problems let me know and I can maybe help with the code
