the approach I suggested would require you to create a separate user
account for every user that you wish to be able to take the survey.
As of today I don't think you can limit users access by sending a cookie. Even
if you could, what about shared computers, or it is not a concern for your 
target market.
Anyway it is possible to add this feature, but it's up to James Flemer to make a
decision on that. Hopefully he'll read the post and you'll hear what he has to
say about it soon.
