Initial Comment:
I've read all the documentation and the mailing 
list archives and the instructions in the files, etc.

I'm SO stuck on this question:
Is it possible to have the user signup for a 
username and password that will allow them to 
partially fill out the survey, save it and then come 
back later, login and complete the survey?

Is this even possible????

I see instructions about the handler-prefix.php 
and the hander.php, but I can't figure out how to 
use this in a PHP file.
